Important CRB information

It will soon be time for us all to fill in new CRB forms as we all did them about three years ago. The form has changed a bit to simplify things this time. Here are a few guidance notes. ECB will automatically send you a new form when your old one is due to run out, so you don't have to worry about it expiring. Should something go astray or you muck one up, I have a huge stack of spare ones.

• USE BLACK INK. Going over blue ink or pencil with a black pen is not acceptable any more.
• Don't put a space in postcodes, National Insurance Ids or telephone numbers.
• DON'T USE TIPPEX. If you make a mistake, cross it out and put the correct thing to the right of the box.
• Use an "X", not a tick in boxes that require them.
• Write one letter in each box, and don't go outside the boxes.

Section A

Fill everything in. Make sure part 9 is "mmYYYY" format, so if you have lived in your house since March 2001, use "032001".

Section B

Part 13 is VOLUNTEER UMPIRE

Part 14 is ECB

Part 15 is the ECB address from the top of the form.

Section C

Only fill in parts 20-27 if you have changed your name (maybe on marriage). Fill in all parts 28-35.

Section D

Only fill it in if you have not lived in your house for five years. If you have moved more than once, put the previous house in here and the others on a separate sheet. Make sure there are no gaps. If you have lived permanently abroad in the last five years, contact me.

Sections E, F, G

Must be left blank. That saves a bit of time. No need for a referee any more.

Section H

Fill everything in. Make sure you sign (item 68) and date it (item 70).

Section X

Leave blank. I will fill it in. I only need two from

• Passport
• Driving License
• Birth Certificate
• Valid Photo Id

and one from

• Marriage Certificate
• Bank Statement
• Car Registration Document
• Utility Bill

So if you have a modern driving license with your photo on it and a car registration document with your address on it, that'll do. Or maybe a passport, old-style license and a phone bill with your address on it.

Sections Y & Z

Leave blank.

CRBs can be filled in by Bushy or Frank. We will do anyone's CRB at any CCUSA meeting if you bring us the stuff. We cannot do them by post. It still takes 4-6 weeks for a new one. When you get it, phone or email Frank with your new number and issue date. CCUSA cannot accept out-of-date CRBs. No CRB, no appointments.
